Upstate Orthopedics has compiled the following information to help patients find what they need quickly. From here, you can pay your bill online, pre-register for your appointments, complete a satisfaction survey, request appointment changes, and find insurance information.
Appointments are scheduled Monday through Friday from 8 am to 4:30 pm. To schedule an appointment with one of our physicians, please call our main line (315) 464-8600. If you do not have a specific physician referral, we will do our best to obtain the earliest available appointment with the physician most familiar with your type of problem/injury. You may also complete an online appointment request and someone will get back to you.
If you are unable to keep your appointment, please give us at least 24 hours’ notice.
We make every effort to honor all time commitments. Unfortunately, because of unanticipated problems and emergencies, your appointment may sometimes be delayed. We will make every effort to keep such delays to a minimum and will keep you informed of them as they occur.
Our goal is to provide you with the best possible medical care. Because this is a teaching facility, residents and medical students may be involved in your care under the direct supervision of your doctor. You may also be seen by a nurse practitioner or physician assistant when the physician is unavailable.
New Patients
If you have any pertinent records, such as X-rays, procedures, surgeries, etc., call your corresponding physician’s office to request that a copy of these records be sent to our office. This additional information is very important to you and our physicians and will assist them in determining the treatment plan most appropriate for you.
Prior to your appointment you may want to make sure of the following:
- You have obtained any necessary referrals or authorizations
- You bring any necessary insurance forms and insurance identification cards with you
- You have requested medical records and/or radiology films from other physician offices and they have been sent to our office
- You have a pre-written list of questions you may want to ask your doctor
- You pre-register using the online pre-registration portal or call (315)464-8600 (insurance and demographic information)
- If it becomes necessary to cancel this appointment, please call the office to let us know as soon as possible.
Patients are encouraged to pre-register to simplify the check-in process on the day of the appointment. You can pre-register by phone or online. To pre-register by phone, please call (315) 464-8600 or 1-877-888-7598 (toll free) at least 3 business days prior to your appointment.

Online Appointment Pre-Check In has arrived! Pre-Check In is a simple and efficient way to check in ahead of time for your upcoming appointment from your phone or computer, at home or at work. If you have an active MyChart account, simply log in to access your upcoming appointment, click on the pre-check in button and follow the simple steps to complete the pre-check in process. Upon completion, you will receive a QR code (barcode) to scan at our Self –Check In Kiosks the day of your appointment. Just answer a few questions at the Kiosk and your Check-In is complete! If you don’t currently have a MyChart Account, please contact our office to receive information on how to sign up for MyChart and start using Pre-Check In to “Save Time and Skip the Line!”

Although we currently accept a wide range of insurance plans, we do not participate with all plans. To confirm whether or not we participate with your plan, contact our billing department at (315) 464-5551.
Patients are responsible for obtaining required referrals from their primary care provider or insurance carrier prior to their appointment. Please be sure to bring ID and all insurance cards with you to each appointment.
Copays are due at the time of service unless other arrangements were made with our billing department prior to your appointment. We accept cash, checks, Visa, MasterCard, and Discover.
To assist patients without health insurance (self-pay) we offer a 30% reduction if payment in full is remitted at the time of service. If this is not feasible, we also offer financial hardship adjustments. To obtain an application, please inquire when you check-in for your appointment or contact our billing department at (315) 464-5551.
This service allows you to quickly and conveniently pay your Upstate Orthopedics bill. Payment in full is requested upon receipt of your statement. We accept payments using the following credit cards: Visa, MasterCard, and Discover.
If you have any questions regarding your Upstate Orthopedics account, you may contact a billing representative at (315) 464-5551. Representatives are available Monday through Friday, 8 am - 4:30 pm.
If your insurance is an HMO/managed care plan and you are required to obtain a referral from your primary care physician, the referral must be obtained before you can be seen for an office visit.
If your insurance is United Healthcare MEDICAID MANAGED CARE plan your primary care physician is required to submit a referral directly to UHC prior to being seen in our office.
Please coordinate with your primary care physician in advance of your appointment. Appointments will be rescheduled if required referrals are not received by the time of your visit.
Prescriptions and refills for established patients of Upstate Orthopedics are routinely issued during our regular office hours (see above). You can make your request by calling our office at (315) 464-4472. No refills are issued during the evenings, weekends, holidays, or during OrthoNOW! hours. It is helpful to anticipate your refill needs and give 48 hours notice to your physician that a refill is required to avoid an interruption in your medication therapy.
