Appointment Requests

  • Current patients can request an appointment through their Upstate MyChart Patient Portal.
    • To register for MyChart, click HERE.
  • To SCHEDULE, CHANGE, or CANCEL an appointment at either our East Syracuse or Township 5 orthopedic office, click here for our online Appointment Request Form.
  • To schedule an appointment with our Spine Division for back or neck issues, contact our office directly at (315) 464-8620.

Pre-Registration

  • You can easily pre-register for your appointment via MyChart.

Pre-Check In

  • Within 7 days of your upcoming appointment, log in to MyChart or click HERE to confirm your registered information and expedite your check-in process on the day of your appointment.

Please be sure to bring all diagnostic imaging or test results that have been ordered or performed by other physicians to your appointment.
