Appointment Pre-Check-In

  1. Within seven days of your upcoming appointment, access your MyChart Pre-Check In.
  2. Select the upcoming appointment from your homepage under “TO DO.”
  3. If appointment is eligible for electronic check-in, the Precheck-In button will show up next to the appointment.
  4. Click on Pre-Check In.
  5. Verify or Edit information in the following tabs:
    1. DEMOGRAPHICS
    2. INSURANCE
      1. Responsibility for Payment
      2. Active Coverages
    3. MEDICATIONS
    4. ALLERGIES
    5. CURRENT HEALTH ISSUES
  6. Click SUBMIT.

On the Day of Your Appointment, please bring any images (x-ray, MRI, CT) that were taken at outside offices.
